How to Get Started

Pathway to Change serves adults in recovery who are currently incarcerated at Lehigh County Jail. All referrals come exclusively through the jail's case management team — there is no walk-in enrollment for this program.

If you or someone you know is incarcerated at Lehigh County Jail and interested in the program, the first step is to speak with a jail case manager.

Case Management

From housing and medical benefits to SNAP and basic needs, our case managers work alongside participants to clear the path — identifying barriers before release and staying connected once you're back in the community.

Recovery Support Services

Certified Recovery Specialists work one-on-one with participants to build recovery capital, develop relapse prevention plans, and set recovery goals that carry forward into life outside.

Community & Events

Change on Hamilton itself is a resource. Participants are encouraged to use the center — attending job fairs, work readiness seminars, sober social events, wellness activities, and more.
